Use this page only if you have been instructed by our office to submit a payment.

If you are covered by a legal plan (including MetLife, ARAG, or similar plans), please do not submit payment through this page unless:

• Your legal plan has advised that the service is not covered; or

• You have entered into a separate agreement with our office for services that are not covered by your legal plan; or

• Our office has specifically instructed you to submit payment.

If you are unsure whether payment is required, please contact our office before submitting payment.
